Appointment Request

Please use the below form to request an appointment. Please note that the date and time you requested may not be available, and we will contact you to confirm appointment details within 48 hours. If this is an emergency, call our clinic immediately at (734) 878-4302. Do not use this form if you are experiencing an emergency with your pet.

We do need to see your pet on an annual basis to continue a provider relationship with your family. By law we cannot dispense any medications without having seen you within that one year time frame. We do send out annual reminders for your pet’s vaccine needs, call and make an appointment when it arrives.
